Over recent decades, the consumption of medicines, predominantly pharmaceuticals and chemical health products, has increased significantly due to the amplified demand for treatments for a range of illnesses, such as headaches, relapsing fevers, dental issues, streptococcal infections, bronchitis, and ear and eye infections. In contrast, overreliance on these methods can cause substantial environmental deterioration. Despite its frequent use as an antimicrobial medication in both human and veterinary practices, the presence of sulfadiazine in the environment, even in trace amounts, raises the alarm as a potential emergency pollutant. Effective monitoring necessitates speed, selectivity, sensitivity, stability, reversibility, reproducibility, and ease of use. A modified electrode comprising carbon, combined with electrochemical techniques like cyclic voltammetry (CV), differential pulse voltammetry (DPV), and square wave voltammetry (SWV), offers an excellent, efficient, and user-friendly method. This choice simplifies control, accelerates analysis, and protects human health from the accumulation of drug residues. A comparative analysis of chemically modified carbon-based electrodes, including graphene paste, screen-printed electrodes, glassy carbon, and boron-diamond doped electrodes, is presented for the detection of sulfadiazine (SDZ) in various matrices such as pharmaceutical formulations, milk, urine, and feed samples. Recent advancements in the academic fields of prosthetics and orthotics (P&O) have spurred a surge in scientific research within this discipline. Nevertheless, the quality of published research, especially randomized controlled trials, does not always reach the desired level of acceptability. To that end, the study endeavored to evaluate the methodological and reporting standards of randomized controlled trials in the Iranian P&O field, with the goal of identifying existing shortcomings.
Six electronic databases, namely PubMed, Scopus, Embase, Web of Science, the Cochrane Central Register of Controlled Trials, and the Physiotherapy Evidence Database, underwent a thorough search from January 1, 2000, to July 15, 2022. The Cochrane risk of bias tool served to evaluate the methodological quality of the studies that were part of the analysis. Using the Consolidated Standards of Reporting Trials (CONSORT) 2010 checklist, the reporting quality of the included studies was determined.
We scrutinized 35 randomly controlled trials, published between the years 2007 and 2021, as part of our comprehensive analysis. Evaluating the methodological quality of 18 RCTs revealed a significant deficiency, contrasting markedly with the superior quality of 7 studies and a satisfactory level of quality in 10 additional studies. A median score of 18 (13–245) out of 35 was found for the reporting quality of RCTs, when evaluated by the CONSORT items. The examination of relationships revealed a moderate correlation between the CONSORT score and the publication year of the RCTs that were incorporated. Despite this, a weak relationship existed between CONSORT scores and the impact factors of the journals.
RCTs in Iran's P&O sector fell short of optimal methodological and reporting standards. To bolster the methodological soundness, a more rigorous approach is necessary regarding items like masking outcome assessments, concealing allocation methods, and creating random sequences. The current study's purpose is to identify the presence of TORCH infections in a child with both bilateral cataracts and deafness, including a report of the ToRCH serology screening (Toxoplasma gondii [TOX], rubella [RV], cytomegalovirus [CMV], and herpes simplex virus [HSV I/II]) findings specific to the pediatric population with both cataracts and hearing loss.
Cases in the study possessed a clinically evident history of congenital cataracts and congenital deafness. AIIMS Bhubaneswar admitted 18 children with bilateral cataracts and 12 children with bilateral deafness for cataract surgery and cochlear implantation, respectively. All children's sera were sequentially examined for qualitative and quantitative IgG/IgM antibody responses to TORCH agents.
Detection of anti-IgG antibodies directed against the torch panel was observed in every patient presenting with both cataract and deafness. Bilateral cataract children, 17 of 18, and bilateral deaf children, 11 of 12, demonstrated the presence of anti-CMV IgG in their blood samples. Statistically significant elevations were observed in the rates of anti-CMV IgG antibody positivity. A significant proportion of cataract patients, specifically 94.44%, were found to have positive Anti-CMV IgG, while 91.66% of patients in the deafness group also exhibited this positivity. Apart from that, 777% of the cataract group and 75% of the deafness patients were positive for anti-RV IgG antibodies. Among bilateral cataract patients with positive IgGalone, the most common pathogen was Cytomegalovirus (CMV) (17/18, 94.44%), followed by Rhinovirus (RV) (14/18, 77.78%). Human Herpes Viruses, HSV-1 (5/18, 27.78%) and HSV-2 (3/18, 16.67%), along with Toxoplasma (TOX, 5/18, 27.78%) were also implicated. Bilaterally deaf patients exhibiting seropositivity to IgG alone demonstrated a nearly identical spectrum, with the sole exception of TOX (no cases out of 12).
In pediatric cases of cataracts and deafness, the current study highlights the importance of cautious interpretation of ToRCH screening data. Clinical correlation, in tandem with serial qualitative and quantitative assays, should be included in the interpretation to reduce diagnostic errors. The potential for infection transmission necessitates testing older children for sero-clinical positivity.
